There's a lot of people on hand for this surgery, taking place in the Brooklyn Navy Yard not far into the Twentieth Century. The patient's leg is being held in the air and there is an ether cone on his face. On close inspection, it looks like the doctor is approaching the leg with a scissor. He's apparently not ready to use the ghastly-looking saw on the table behind him. However well this surgery went, the patient would have had a fair chance of sepsis. Not one person in this operating room is wearing a mask or gloves. Let's hope they at least washed up first.
